Licensing/Certification Requirements

In order to become a licensed massage therapist in the state of Kentucky, you will need to fully complete the application process.

In addition to your application you will also be asked to submit the following documentation:

  • To apply for licensure, you must print and completely fill out the Initial License Application found on the website.  You can follow the link here, which will take you directly to the application.
  • The Board requires an official copy of your transcripts from the massage school that you attended.  These transcripts must be mailed directly to the Board by the school in a sealed envelope.  A broken seal on an envelope will make the transcripts “unofficial”.
  • Submit a criminal background check.
  • Attach documentation if you have ever been licensed in another state or jurisdiction.
  • Reasons for application denial in the past, if applicable.
  • Enclose a nonrefundable check of $125 made payable to Kentucky State Treasurer.
  • Please mail all documents and your application to the following address:

Kentucky Board of Licensure for Massage Therapy
P.O. Box 1360
Frankfort, KY 40602

Requirements to Start a Massage Business in Kentucky (Massage Establishment License)

Starting a massage business in the state of Kentucky is a very exciting and exhilarating process.  To begin, you will want to utilize all resources available to you in the state.  The following are some topics and resources that you might find useful in starting your business:

  • Kentucky One Stop Business Portal
  • Choose a name and structure for your business
  • Register your business with the state
  • File for tax numbers and IDs
  • Check local requirements, including licenses
  • Obtain your massage therapy license
  • Choose a location and check zoning permits

Massage License Renewal Process

To begin the renewal process of your massage therapy license, you must document and complete 24 hours of continuing education credits.  Of the 24 total credits, you should obtain at least 3 hours of ethics.  The documentation of these continuing education courses should include:

  • The Course title
  • The Instructor’s name
  • The date attended
  • The hours earned
  • The Provider’s number

Because it is illegal in the state of Kentucky to practice massage therapy with an expired license, it is critical that you renew your license in time.  The Kentucky law states that if your renewal is 1 day late, a $150 late fee will be administered to your application.  After 90 days, your license will be terminated if you have not renewed.

You must complete and mail your renewal form directly to the Board; the online renewal software has been suspended because there were issues associated with it.  To locate the renewal application, please visit and under the “Resources” tab you will find the “Applications and Forms”.

Here you will find the printable version of the application form.  Enclose all fees and mail it to the following address:

Division of Occupations & Professions
P.O. Box 1360
Frankfort, KY 40602

Massage License Reciprocity

If you have a license from a state or jurisdiction other than Kentucky, then you may be eligible for reciprocity.  Check with the Board directly about the limits and requirements for applying by endorsement.  You may need to prove verification of your license and educational background depending on where you are moving.

Note: Specific personnel information, office addresses, fees, payments etc. may change over time. Although we do our best to keep this site up to date, please ensure you contact the state board to obtain the most recent information at the time of your application.

Here is a list of the top massage therapy schools near you. We have an arrangement with all of them that allow you to request information for FREE. Please do so and take that first step toward a better future.

Where can I find licensed individuals who are seeking employment? I am looking for a massage therapist, specializing in perinatal massage in the Ashland, KY area, to be a part of a growing multi service business.
